The Marine Hospital Fund was established by Congress in 1798 as a network of hospitals that cared for ill and incapacitated seamen.  The fund was reorganized in 1870 as the military Marine Hospital Service which later became the U.S. Public Health Service.  Surgeon generals are nominated by the United States president and are then confirmed by the Senate and serve a four-year term.
